What really happens at the end of The Giver by Lois Lowry?

Does Jonas actually find a sled and ride down the hill to a waiting, loving community? If so, how did the sled get there? Or is it all symbolic and Jonas and Gabriel actually die? Is Elsewhere an actual place or symbolic of heaven? Thanks.
